The Registered Nurse (RN) Case Manager provides compassionate, patient-centered care by coordinating all aspects of clinical services for patients and families receiving hospice care. This role serves as the primary point of contact for each assigned patient, developing and implementing individualized care plans in collaboration with the interdisciplinary team (IDG). The RN Case Manager ensures the delivery of quality clinical care that meets regulatory requirements, organizational standards, and the unique physical, emotional, and spiritual needs of each patient and family.
Core Responsibilities:
- Assessing, planning, implementing, and evaluating the nursing care of hospice patients in accordance with the plan of care and hospice philosophy.
- Performing comprehensive initial and ongoing patient assessments, documenting accurately and promptly in the EMR.
- Coordinating all aspects of patient care, including clinical visits, medications, equipment, and supplies.
- Collaborating closely with physicians, social workers, chaplains, and other members of the interdisciplinary team to develop and update individualized plans of care.
- Providing education and emotional support to patients and families regarding disease progression, symptom management, and end-of-life care.
- Monitoring and managing patient symptoms, administering prescribed medications, and ensuring comfort and dignity.
- Participating in on-call rotation as assigned to provide after-hours nursing support and crisis intervention.
- Ensuring all documentation meets compliance standards, regulatory requirements, and agency policies.
- Maintaining open communication with patients, caregivers, and team members to promote continuity and quality of care.
- Supporting the bereavement process by identifying family and caregiver needs and making appropriate referrals.
- Participating in quality assurance, performance improvement, and interdisciplinary team meetings.
- Demonstrating leadership, professionalism, and accountability in all aspects of patient care delivery.
Basic Qualifications/Requirements:
- Graduate of an accredited school of nursing; bachelor’s degree preferred. Current Registered Nurse (RN) license in the state of practice.
- Minimum of two (2) years of clinical nursing experience; hospice or home health experience preferred.
- Strong clinical assessment, critical thinking, and case management skills.
- Proficient in documentation and use of EMR systems, MatrixCare preferred.
- Demonstrated ability to work independently and as part of an interdisciplinary team.
- Excellent interpersonal communication and problem-solving skills.
- Valid driver’s license, reliable transportation, and auto insurance in accordance with state requirements.

Pay: $87,500.00 - $95,000.00 per year
