We are seeking a dedicated and energetic Physician (MD/DO) who is boarded in Internal Medicine or Family Medicine specializing in hospital medicine and skilled in providing comprehensive care within Skilled Nursing Facilities (SNF) and Assisted Living Facilities (ALF). This vital role involves delivering high-quality inpatient and post-acute care, ensuring smooth care transitions, and enhancing patient outcomes through expert clinical judgment. The Physician will collaborate closely with multidisciplinary teams to provide compassionate, efficient, and evidence-based medical services in diverse healthcare settings. About the role
We are a growing medical group based in New York seeking a full-time, board-certified or board-eligible MD or DO to join our team in a dynamic dual-coverage role. This position offers the ideal blend of inpatient hospital medicine and post-acute care, allowing you to work across two care settings in a structured, alternating weekly schedule. Duties
Manage acute and chronic conditions across inpatient, outpatient, and post-acute care environments, utilizing advanced clinical skills and evidence-based protocols.
Coordinate with nursing staff, specialists, therapists, and case managers to develop comprehensive care plans
Utilize electronic health record (EHR) systems such as Epic or other EMR platforms to document patient encounters accurately, track progress, and ensure compliance with HIPAA regulations.
Participate in discharge planning
Qualifications
Board Certified in Internal Medicine or Family Medicine
Medical degree (MD or DO) from an accredited institution with active licensure to practice medicine.
Proven experience in hospital medicine or post-acute care environments such as SNF or ALF settings.
Strong knowledge of inpatient clinical
Experience managing complex cases involving geriatrics, cardiology, endocrinology, pulmonology, infectious disease care, psychiatry, or rehabilitation medicine is highly desirable.
Excellent communication skills for patient service coordination and multidisciplinary collaboration.
Ability to perform comprehensive physical examinations and develop individualized treatment plans efficiently.
Prior experience working within hospital settings

Pay: $275,000.00 - $325,000.00 per year
