We are seeking a dynamic and dedicated Registered Nurse Case Manager to join our compassionate team. In this vital role, you will coordinate comprehensive patient care, facilitate effective discharge planning, and ensure seamless communication across multidisciplinary teams. Your expertise will drive positive outcomes and elevate the standard of home health and hospice care delivery. Responsibilities
- Conduct thorough patient assessments, including vital signs, physiology evaluation, and medical record review to determine care needs.
- Develop personalized care plans aligned with patient goals and medical requirements.
- Manage utilization review and case management activities, including intake procedures and diagnostic evaluations.
- Monitor patient progress through ongoing assessment of vital signs and medical status; adjust care plans accordingly.
- Collaborate with interdisciplinary teams to facilitate patient service excellence, including infectious disease care and geriatrics considerations.
- Requirements
- Valid RN license with current registration in the applicable state.
- Strong knowledge of medical terminology, physiology, and acute care procedures.
- Proven ability in patient assessment, discharge planning, and utilization management.
- Familiarity with medical records systems and documentation standards compliant with HIPAA.
- Excellent communication skills with a focus on compassionate patient service and team collaboration.
