Overview / Purpose
The role delivers wraparound case management in the field (homes, shelters, streets, hospitals, courts) to engage hard-to-reach clients, stabilize them in the community, reduce hospitalizations and incarcerations, and connect them to ongoing treatment and resources.
Core Responsibilities
- Conduct outreach and engagement with clients in their natural environments
- Complete assessments of psychiatric, medical, social, housing, and substance use needs
- Develop, implement, and update individualized treatment/recovery plans (ITRPs) in collaboration with the clinical team
- Coordinate access to psychiatry, therapy, medication management, primary care, and community resources
- Monitor medication adherence and symptom changes; support crisis intervention and de-escalation
- Assist with benefits, housing, entitlements (Medicaid, SSI/SSDI), transportation, and vocational services
- Accompany clients to appointments and advocate on their behalf
- Maintain timely, accurate documentation, progress notes, and encounter logs for billing/compliance
- Collaborate with multidisciplinary team members, hospitals, courts, and family supports
- Provide some after-hours or on-call crisis availability depending on program model
Qualifications
- Bachelor’s degree in social work, psychology, human services, or related field (some roles accept relevant experience or require a master’s)
- Case management or behavioral health experience, ideally with SMI/SUD populations
- Knowledge of community resources and the local behavioral health system
- Valid driver’s license and reliable transportation (fieldwork is central)
- Strong crisis management, documentation, and interpersonal skills
- Credentials such as CPRP, CAC/CSC-AD, or state-specific certification may be preferred
Work Environment Primarily field-based with significant local travel, variable hours, and exposure to high-acuity situations.
Pay: $30.00 - $35.00 per hour
Work Location: Hybrid remote in Denton, MD
