Overview
Join our dedicated healthcare team as a Home Health Certified Nursing Assistant (CNA), where your compassionate care and attention to detail will make a meaningful difference in the lives of individuals receiving home health services. In this vital role, you will provide personalized support to patients across various settings, including assisted living, hospice, and long-term care environments. Your expertise in patient observation, medication administration, and assisting with Activities of Daily Living (ADLs) will ensure our clients receive safe, respectful, and high-quality care. This position offers an engaging opportunity to work closely with diverse populations, including seniors, individuals with disabilities, and those managing complex health conditions like CHF, COPD, DM, Dementia, Alzheimer's disease and Post-op patient's.
Duties
- Perform thorough patient observations to monitor health status and report changes promptly
- Assist with ADLs such as bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts and wheelchairs
- Take vital signs including blood pressure, pulse, temperature, and respiration rates to track patient health trends
- Support patients with specialized needs such as catheter care, dementia and memory care, and Alzheimer's care
- Utilize Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems for documentation of patient care activities and medical records
- Conduct light housekeeping tasks such as laundry and cleaning to promote a safe and comfortable living space
- Work collaboratively with nursing staff and other healthcare professionals to ensure comprehensive patient care plans are followed
- Support patients’ emotional well-being through compassionate communication and engagement during visits
Experience
- Prior experience working in home health or community care environments is highly preferred
- Demonstrated knowledge of medical terminology relevant to caregiving roles
- Experience providing senior care, long-term care, or working with individuals with developmental disabilities or disabilities is advantageous
- Familiarity with EMR/EHR systems for accurate medical documentation and record keeping
- Certification in CPR (Cardiopulmonary Resuscitation) is required to ensure readiness for emergency situations
