Speech Language Pathologist (SLP) – PRN Home Health

Position Summary
As a PRN Speech-Language Pathologist (SLP), you will evaluate, develop, and implement individualized treatment plans for patients with speech, language, cognitive, voice, and swallowing disorders. You'll work collaboratively with physicians, nurses, therapists, and caregivers to help patients achieve the highest possible level of independence and quality of life.
Responsibilities
- Perform comprehensive speech, language, cognitive, and swallowing evaluations.
- Develop and implement individualized treatment plans based on physician orders.
- Provide skilled speech therapy services in the home setting.
- Educate patients, families, and caregivers on treatment techniques and home exercise programs.
- Collaborate with physicians and the interdisciplinary care team to coordinate patient care.
- Document evaluations, treatment sessions, progress, and outcomes accurately and within required timeframes.
- Participate in case conferences and quality improvement initiatives as appropriate.
- Maintain patient confidentiality and comply with all agency policies and regulatory requirements.
- Serve as case manager for therapy-only patients when applicable.
Qualifications
- Master's Degree from an accredited Speech-Language Pathology program.
- Current Florida Speech-Language Pathologist license.
- Minimum one (1) year of Speech Therapy experience (home health experience preferred).
- Current CPR certification.
- Valid Florida driver's license, reliable transportation, and current auto insurance.
- Strong communication, documentation, and organizational skills.
- Ability to work independently while collaborating effectively with an interdisciplinary team.
