Community Health and Rehabilitation Center is looking for a SLP!
Applicants must have a current SLP license in the state of Florida in good standing, and must be ASHA Certified. At least one year of experience in a long term care setting is preferred.
Essential Job Duties are as follows, but are not limited to,
- Assessing functional levels and capabilities of residents swallowing and speech, as well as make make recommendations related to residents therapy treatments
- Provides speech therapy treatments by following residents plan of care
- Develops goals which are measurable, and realistic based on residents care plan
- Participates in discharge planning process
- Thoroughly documents treatment progress in residents charts accurately and timely as well as completes required company forms and documents to ensure compliance with state and federal regulations
- Conducts, rates, and interprets hearing and speech evaluations and tests
- Maintains positive working relationship with residents and their families, as well as provide them with any training and education for speech therapy and proper speech monitoring techniques
- Preforms overall supervision to SLP- CFYs and rehabilitation aides to ensure quality rehab care services
- Helps assist residents to the gym, schedule appointments, take calls, reorder supplies and completes insurance form
