Family Choice is a therapist owned and operated private company seeking Physical Therapists. We are seeking highly motivated, energetic and dynamic contract Physical Therapists in the rehab/nursing home setting.
This is a contract position which allows the right candidate, flexibility and freedom to select caseload size, hours and to make their own schedule. Great fit for therapists who want independence or need flexibility to accommodate personal schedules. We have a wonderful team and are looking forward to expanding. our reach to the following counties: Fayette, Clark, Madison, Franklin, Jessamine
Responsibilities:
- Provision of quality physical therapy care within the PT scope of practice in accordance with established standards of practice and departmental procedures
- Evaluation and identification of patients who qualify for services
- Therapeutic intervention to all qualified patients
- Establishment of short-term goals, long-term goals within the treatment plan
- Completion of all documentation in a timely manner
- Possibly supervision of Physical Therapy Assistants
Qualifications
- Able to work independently
- Licensed Physical Therapist in the State of Kentucky
- Previous Home Health Experience

Pay: $70.00 - $75.00 per hour
