Responsibilities
- Conduct thorough assessments of patients’ physical conditions, including injury evaluation and mobility limitations
- Develop and execute individualized care plans tailored to each patient’s needs, goals, and medical history
- Provide hands-on therapy techniques such as trigger point therapy, orthopedics-focused interventions, and geriatrics-specific treatments
- Collaborate with multidisciplinary teams to coordinate discharge planning and ensure seamless transition across inpatient, outpatient, or home health settings
- Educate patients and their families on physiotherapy exercises, injury prevention strategies, and community resources
- Maintain accurate documentation within electronic medical record (EMR) systems, including progress notes and treatment updates
- Stay current with medical terminology, anatomy knowledge, and physiotherapy advancements to deliver evidence-based care
Experience
- Proven experience working in inpatient hospital environments with exposure to acute care scenarios
- Demonstrated success in outpatient clinics or home & community care settings delivering patient-centered physiotherapy services
- Strong background in discharge planning, care plan development, and patient education across diverse populations such as pediatrics and geriatrics
- Familiarity with EMR systems for documentation and treatment tracking
- Knowledge of physiology principles related to musculoskeletal injuries, neurological conditions, and post-surgical rehabilitation
- Experience applying trigger point therapy techniques for pain relief and muscle relaxation
- Prior work in hospital or home health environments emphasizing patient care excellence in both clinical and community contexts
Pay: $130,000.00 - $140,000.00 per year
