Job Summary
We are seeking a compassionate and dedicated Licensed Vocational Nurse (LVN) to join our healthcare team. The ideal candidate will provide high-quality nursing care to patients in a skilled nursing facility setting. Duties
- Administer medications and treatments as prescribed by physicians.
- Assist in case management and discharge planning for patients.
- Provide home care services, including personal hygiene assistance and monitoring of vital signs.
- Document patient information accurately in medical records using Point Click Care
- Perform catheterization and other clinical procedures as necessary.
- Educate patients and families about health management and botulinum toxin treatment options.
- Support the care of children with special needs or chronic conditions.
Skills
- Strong knowledge of case management principles and practices.
- Proficiency in primary care nursing practices.
- Ability to perform botulinum toxin treatments safely and effectively.
- Skilled in discharge planning processes to facilitate smooth transitions for patients.
- Competence in medical documentation to maintain accurate patient records.
- Proficient in catheterization techniques and patient assessments.
- Experience using eClinicalWorks for electronic health record management.
