JOB SUMMARY
The Orange VNA is currently hiring for Part-Time Home Health RN Admission Nurse for the small territory of Orange, Milford, West Haven and Woodbridge. Small case load, about 2-3 Admissions a day.
RESPONSIBILITIES
- Provides supervision and oversight to patient care as outlined by State regulation and/or requested by Agency Director.
- Understands the concepts of home health care and the role of the RN as a member of a full discipline health care team
- Provides skilled hands on care to patients in the comfort of their own home
- Performs start of care visits, resumption of care visit and revisits with timely documentation
- Develops individualized Plan of Care for patients
- Provides education to the patient and their family/support person
- On call rotation with other RN team members
Experience/License Requirements
- Minimum of 1 years in Home Care
- Must have a current RN license in CT
- Current drivers license and valid car insurance
