Paid Family Provider (Arizona DDD Program)
Location: Arizona
Pay: $16.00–$18.00 per hour
Schedule: Part-Time, Full-Time, Flexible

Responsibilities
Provide care for your own loved one, including:
- Personal care and ADLs
- Meal preparation
- Companionship
- Community activities
- Supporting daily routines
- Required documentation
Requirements
- 18 years or older
- Arizona resident
- Currently caring for your own loved one with a qualifying disability
- Pass a background check
- Complete required training
- Become an approved DDD Paid Provider (we'll help you through the process)
