Job Description:

Provide for activities of daily living by assisting with serving meals, feeding patients as necessary, and ambulating, turning, and positioning patients.

Provide fresh water and nourishment between meals, bathing and showering for residents.

Maintain patient stability by checking vital signs and weight.
- Test urine and record intake and output information.
- Provide patient comfort by utilizing resources and materials.
- Transport patients as needed.
- Answer patients’ call lights and requests.
- Report observations of the patient to the nursing supervisor or charge nurse.

Document actions by completing forms, reports, logs, and records.
- Maintain work operations by following policies and procedures.
- Protect the organization’s value by keeping patient information confidential.
- Update job knowledge by participating in educational and professional opportunities, reading professional publications, and maintaining licensure.
- Enhance nursing department and facility reputation by accepting ownership for accomplishing new and different requests.
