About the Role:
As our RN Case Manager in Round Rock, you'll be the heart of the care team, coordinating personalized hospice services that bring comfort, dignity, and peace to our patients and their loved ones. You’ll work closely with physicians, social workers, CNAs, chaplains, and families to ensure everyone is supported — emotionally and clinically.
What You'll Be Doing:
- Managing a caseload of hospice patients in the North Austin CTX area
- Conducting in-home visits and assessments
- Creating and updating individualized care plans
- Providing education, comfort, and compassionate support to patients and families
- Collaborating with a multidisciplinary team (and yes, we actually enjoy working together!)

What You Bring:
- Current RN license in the state of Texas
- Hospice or home health experience (preferred, but not required — we’re happy to train the right heart!)
- Strong communication and organizational skills
- Valid driver’s license, and reliable transportation
Location: Round Rock, TX & surrounding areas
Schedule: Full-time
