We provide geriatric outpatient therapy at HOME. Our therapists can provide patients with Physical and Occupational Therapy where it’s most convenient for them. There are no waiting rooms, no time wasted on travel, and no need to arrange rides. Patients get all the benefits of an in-person, 1:1 visit without the hassle of going to a clinic.

- Monitoring vitals and medications

Our Outpatient Therapy company is seeking an Occupational Therapist for at-home therapy visits. Our patients are generally geriatric and can benefit from consistent therapy (at home) from reliable, compassionate, creative individual(s). The ideal candidate is proficient at analyzing patients' home environments and creating effective treatment plans based on their impairments and unique needs.
Responsibilities:
- Conduct patient evaluations
- Schedule follow-up visits for patient treatment (**AT THE THERAPIST'S CLINICAL DISCRETION for suggested frequency and duration for plan of care**)
- Be a leader in their treatment plan and communicate/refer to interdisciplinary professionals as indicated
- Communicate with/train family member(s) or caregiver(s) to ensure the upmost health benefit and maximized functional mobility of their patients
Job Types: Part-time, Contract
Pay: $65.00 - $75.00 per hour
