VNA of Ohio is one of Northeast Ohio's most established home health and hospice agencies — 124 years strong, CHAP-accredited, and 4-star rated by CMS. We're looking for an experienced RN leader to join our Hospice team as Clinical Supervisor — splitting time between direct patient care in the field on the East Side of Cleveland and clinical oversight and operations in the office. This is a hands-on leadership role for someone who wants to stay connected to patient care while growing into a broader supervisory capacity, with a $2,500 sign-on bonus and top-quartile pay.
Compensation
- $39.00 – $46.00 per hour, based on experience
- $2,500 sign-on bonus
- Full-time / 40 hours per week
Service Area & Schedule
East Side Cleveland | Monday – Friday, standard business hours | Hybrid field and office-based.
What You'll Do
In the Field (~50% of time):
- Conduct hospice visits with patients in their homes on the East Side of Cleveland in accordance with the established plan of care
- Perform skilled nursing assessments and monitor patient symptoms, comfort levels, and changes in condition
- Administer medications and treatments focused on comfort, symptom management, and quality of life
- Support goals-of-care conversations and provide education and emotional support to patients and families navigating end-of-life care
- Conduct joint visits with field staff to support performance, competency, and care quality
- Maintain accurate and timely documentation in the EMR within required timeframes
In the Office (~50% of time):
- Provide clinical oversight and supervision to Hospice field staff including RNs, LPNs, aides, and support personnel
- Monitor caseloads, patient outcomes, and quality indicators across the Hospice service line
- Support onboarding, orientation, and ongoing training and development of clinical staff
- Serve as a clinical resource and subject matter expert for the interdisciplinary team
- Collaborate with leadership to develop and refine hospice protocols, workflows, and best practices
- Ensure compliance with Medicare, Medicaid, CHAP accreditation standards, and agency policies
- Participate in quality assurance, performance improvement, and regulatory readiness activities
- Assist with on-call coverage and after-hours clinical support as needed

Requirements
Must-Have:
- Active RN license (Ohio) + current BLS certification
- Bachelor's degree in Nursing (BSN) required; MSN preferred
- Minimum 3 years of clinical nursing experience in hospice or a related setting
- Minimum 1 year of clinical leadership or supervisory experience
- Reliable vehicle and valid driver's license with ability to travel throughout the East Side Cleveland service area
- Familiarity with Medicare and Medicaid hospice regulations and CHAP accreditation standards
Preferred / Nice-to-Have:
- CHPN certification or willingness to pursue
- Prior experience supervising interdisciplinary hospice teams
- Experience with quality assurance, performance improvement, or regulatory compliance
- Familiarity with HCHB or similar home health/hospice EMR
- Strong coaching, mentoring, and team development skills
Pay: $39.00 - $46.00 per hour
