Position Summary
Luna Vista Hospice is seeking an experienced Hospice Clinical Manager to oversee daily clinical operations and support our interdisciplinary team in delivering high-quality hospice care. This leadership role is responsible for coordinating patient admissions, facilitating interdisciplinary group (IDG) meetings, overseeing clinical documentation, and ensuring regulatory compliance and quality improvement initiatives.
The Clinical Manager plays a key role in supporting staff, maintaining efficient workflows, and ensuring patients and families receive timely, compassionate care.
Schedule: Monday – Friday, 8:00 AM – 6:00 PM
On-Call Rotation: Every 4-6 weeks
Key Responsibilities
- Conduct and facilitate Interdisciplinary Group (IDG) meetings to review patient care plans and coordinate services.
- Oversee and facilitate new hospice admissions, including coordination of medications, durable medical equipment (DME), and patient transportation.
- Review and monitor clinical documentation to ensure accuracy, quality, and Medicare compliance.
- Lead Quality Assurance and Performance Improvement (QAPI) initiatives.
- Assist with staff scheduling and operational coordination.
- Conduct new hire interviews, onboarding, and clinical staff training.
- Coordinate and order patient supplies and equipment as needed.
- Provide guidance and support to clinical team members to ensure best practices in hospice care.
- Collaborate with physicians, nurses, social workers, and spiritual care providers to ensure comprehensive patient care.
- Maintain compliance with Medicare hospice regulations and agency policies.
Qualifications
- Active Registered Nurse (RN) license in the state of New Mexico.
- Hospice or palliative care experience strongly preferred.
- Previous clinical leadership, case management, or supervisory experience preferred.
- Knowledge of Medicare hospice regulations and documentation standards.
- Strong organizational, leadership, and problem-solving skills.
- Excellent communication and interpersonal abilities.
- Experience with electronic medical records (EMR) systems.
- Ability to manage multiple priorities in a fast-paced healthcare environment.

Work Location

Pay: $95,000.00 - $105,000.00 per year
