Job Purpose/Summary:
The Targeted Case Management program is a community-based, mental health program that will provide services to eligible children, adolescents, and adults residing in Westmoreland County. The Enhanced Targeted Case Manager (TCM) will work with acute individuals with a diagnosis of severe and persistent mental illness (e.g., major depressive disorder, schizophrenia, bipolar disorder, severe emotional disturbance, etc.) who experience complex needs and are high utilizers of resources. The position provides professional coordination of mental health services according to the consumer’s and/or family’s level of needs within the Westmoreland County community setting. The incumbent will provide professional assessment, service planning, coordination of services, referral, and re-evaluation of services required for a safe and healthy community life.
The Enhanced TCM serves as a key member of the treatment/service team, assuring often complex services produce positive outcomes. The position is the central contact point in the system, coordinating care and assisting the consumer in their recovery process.
Minimum Qualifications:
Education: Bachelor’s degree in social work, psychology, criminal justice, pastoral counseling, rehabilitation counseling, community mental health, or sociology.
Experience:
· Minimum of 1 year of experience in human services and in the working knowledge of human services paperwork and regulations.
· Case management experience preferred.
Licenses, Certifications & Clearances:
· Act 33/34 Clearances
· FBI Clearance
· Valid State of PA Class 1 Drivers License
· Access to properly insured motor vehicle for client transportation and fieldwork and a safe driving record
Knowledge, Skills, Abilities:

· Knowledge of large systems in Westmoreland County (CYF, probation, MR, forensics, aging, medical).
· Knowledge of child abuse reporting.
· Knowledge of position responsibilities, including state mandates and county expectations.
· Knowledge of systems theory, family systems, family therapy interventions, and psychoeducation.
· Ability to identify and manage the symptoms of severe mental illness.
· Knowledge of medications for managing symptoms of severe mental illness.
· Ability to broker resources and be the “go-to” resource, assuring that there are effective safety nets in place for the individual in services.
· Ability to manage crises.
· Ability to work professionally and effectively with other community agencies, general public, and agency staff.
· Knowledge about identifying and managing substance use problems.
· Knowledge of dual diagnosis (mental health and substance use) treatment, 12-step programs, groups, area resources, and recovery/prevention/rehabilitation programs.
· Ability to identify symptoms, support systems, and community resources.
· Ability to work with children and adolescents who are in placement, receive services of FSS (Financial Support Services), or who need coordination of care following an inpatient stay.
· Knowledge of the needs of the population and community resources (inpatient units, dual diagnosis, intensive outpatient services, etc.).
· Knowledge about identifying and managing co-morbid health problems, as well as promoting wellness.
· Ability to help individuals find meaningful activity, employment, and maintain employment.
· Ability to work autonomously and make independent decisions utilizing knowledge, skills, and good judgment.
Essential Duties & Responsibilities
· Assesses consumer’s strengths and needs through face-to-face, phone, and collaborative contacts.
· Develops a strength-based service plan in conjunction with the consumer and/or family. The service plan will highlight the consumer’s goals and will reflect change over time as goals are met or discontinued.
· Completes risk assessments and effectively manages crises.
· Thoroughly documents services, progress notes, as well as completes appropriate referrals.
· Develops and reviews crisis plans, WRAP plans, and mental health directives.
· Links to natural supports and community and/or specialized services to ensure that the consumer’s needs are met.
· Coordinates and communicates with services across programs/providers.
· Provides advocacy for the consumers and assists with removing barriers, problem resolution, and grievances.
· Convenes/facilitates service planning meetings amongst various providers for coordination of care, ensuring appropriateness and responsiveness of services in relation to the consumer and/or family needs.
· Meets with Supervisor regularly to review expectations, role functioning, case reviews, and all critical matters.
· Abides by all HIPAA, state, and MCO performance standards.
· Participates in after-hours, on-call rotation.
· Meets or exceeds productivity requirements.
