Overview
Join our dedicated healthcare team as a Private Duty Nurse (LPN, RN, BSN), where you will deliver personalized, high-quality nursing care directly to patients in their homes. Duties
- Provide comprehensive nursing assessments and develop individualized care plans tailored to each patient’s needs.
- Administer medications, including injections and IV infusions, following strict protocols and documentation standards.
- Monitor vital signs, perform physical examinations, and evaluate patient progress regularly.
- Manage complex medical procedures such as catheterization, tube feeding, wound suturing, and ventilator care.
- Perform tracheostomy care and suctioning as needed
- Provide nebulizer treatments and airway clearance therapies
- Coordinate with physicians and other healthcare professionals to ensure seamless continuity of care and discharge planning.
- Maintain accurate medical records using EMR (Electronic Medical Records) systems like Epic or Cerner while adhering to HIPAA confidentiality policies.
- Educate patients and their families on health management strategies, medication administration, and safety precautions.
- Educate family and caregivers regarding patient care needs when appropriate
- And any other need as diagnosed by the doctor
Qualifications
- Valid licensure as an LPN, RN, or BSN with current state certification.
- Proven experience in critical care settings such as ICU, emergency medicine, trauma centers (Level I or II), or hospital medicine.
- Strong background in specialized areas including Alzheimer’s care, memory care, hospice, palliative care, Tracheostomy, Nebulizer or geriatrics.
- Familiarity with EMR/EHR systems such as Epic, Cerner, Athenahealth or eClinicalWorks for accurate documentation.
- Experience working with individuals with disabilities or developmental delays; knowledge of ICD-9/10 coding is a plus.
- Proficiency in advanced nursing skills including IV insertion, phlebotomy, sterile technique, airway management and pain control.
- Ability to perform physical assessments and diagnostic evaluations with attention to detail.
- Excellent communication skills for caregiver collaboration and client education.
- Knowledge of long-term care regulations and experience in skilled nursing home care settings is preferred.
