American Traveler is seeking a Physical Therapist for a Home Health position in the East Portland, OR area, requiring at least 1 year of skilled home health experience and Epic EMR proficiency.

Home health setting serving East Portland, OR and surrounding areas
-
1:1 patient ratio with visits to up to 6 patients per day; 30 productivity points is standard
-
8-hour day shifts, Monday–Friday (4x10 and 3x12 schedules cannot be accommodated)

No weekend or on-call requirements

Travelers are expected to float between all facilities within the state; refusal to float may result in termination of the assignment

Charting is done via Epic Remote Client for Home Health (laptop) and Rover phone app/OASIS
-
Candidates within a 50–75 mile radius of the facility are considered local and may be accepted at the same rate

Minimum 1 year of skilled home health experience required

Current BLS certification required

Epic EMR experience required
-
OASIS experience required

Valid PT license required if applicable

Must have reliable personal transportation

Minimum of 2 supervisory references covering 1 year of employment within the last 3 years in a comparable specialty or setting

Full SSN and DOB required for consideration

Business casual attire required; scrubs are allowed but not required and no specific color is mandated; jeans are not permitted

The scheduling system groups clients to minimize drive time; occasional maximum daily radius of roughly 60 miles (60–90 minutes total drive time)

Mileage is reimbursed at the IRS standard rate

First-time travelers are welcome to apply

Former permanent staff must wait at least 3 months before returning as a traveler and cannot be within 50 miles of the submitting facility

Minimal RTO is preferred for this position; a small number of days may be approved
