Short Description

Manage centralized medication processes
-
Completing medication prior authorizations and peer reviews or appeals for medication denials to facilitate provider written treatment plan and improve patient care

Completing centralized medication refills following written protocols that include evaluation of patients chart for appropriate testing and follow up

Manage transitions in care for patients and their care givers

Manage chronic care management as directed by the patient, their care giver or physician/APP

Perform periodic outreach to patients/caregivers and communicate with physician/APPs and RN Care Coordinator on behalf of the patient/ patient care giver

Participate in maintaining and developing quality and performance initiatives for specific patient populations.

Engaged in practice transformation operations. Work with offices to implement performance and quality strategies. Work with team to identify and overcome barriers to implementing transformation strategies.

Other duties as assigned and within the scope of professional licensure as necessary for patient safety. Including redeployment to office for clinical assignments.
