Duties
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary
- Monitor vital signs including blood pressure, pulse, temperature, and respiration to track health status accurately
- Reminder of medications following care plans while adhering to HIPAA regulations to ensure privacy and safety
- Prepare nutritious meals tailored to individual dietary needs and assist with feeding when required
- Conduct patient observation to identify changes in condition or behavior and report findings promptly to healthcare professionals
- Maintain detailed documentation of care provided, medication administration, and patient progress in compliance with healthcare standards
- Support clients with cognitive impairments such as Alzheimer’s disease or dementia through memory care techniques and structured routines
Experience
- Prior experience working in home health care, assisted living facilities, nursing homes, or group homes is highly preferred
- Demonstrated ability to work with individuals with developmental disabilities, autism spectrum disorder, or other special needs
- Knowledge of first aid, CPR certification, and basic nursing skills essential for emergency response and patient safety
- Experience with hospice care or working with patients requiring palliative support is a plus
- Familiarity with care plans development and medication management in compliance with healthcare regulations such as HIPAA
- Strong observational skills for patient monitoring and recognizing signs of deterioration or distress
- Ability to perform heavy lifting safely using equipment like Hoyer lifts when necessary for client transfers
