Position Overview
AlphaCare Home Health is seeking compassionate and motivated Occupational Therapists (OTs) to provide skilled home health occupational therapy services throughout the Bay Area.
In this role, you will evaluate patients in their homes, develop individualized treatment plans, improve functional independence, and help patients safely perform daily activities. You will work closely with physicians, nurses, therapists, caregivers, and other healthcare professionals to deliver coordinated, patient-centered care.
Responsibilities
- Perform occupational therapy evaluations in the home setting
- Develop and implement individualized treatment plans
- Provide skilled occupational therapy interventions focused on ADLs, safety, mobility, and functional independence
- Train patients and caregivers on adaptive techniques, home safety, and durable medical equipment (DME)
- Monitor patient progress and modify treatment plans as appropriate
- Complete timely and accurate clinical documentation
- Coordinate care with physicians and interdisciplinary team members
- Participate in case conferences and care planning
- Support discharge planning and transition of care
- Maintain compliance with agency, Medicare, and regulatory requirements
Qualifications
- Current California Occupational Therapist (OT) license
- Valid California Driver's License
- Strong clinical, communication, and documentation skills
- Ability to work independently in a home health environment
