Evaluate residents and create individualized treatment plans
Provide skilled occupational therapy services focused on maximizing independence and quality of life
Accurately document evaluations, treatments, and progress in Net Health and PCC
Collaborate with nursing, therapy, and interdisciplinary teams
Educate residents and families regarding treatment goals and progress
Maintain compliance with facility and regulatory standards
Current Pennsylvania OT license
SNF/LTC experience preferred but not required
Experience with Net Health and PCC preferred
Strong communication and clinical skills
Positive, dependable, and team-focused attitude
