DEPARTMENT: Activities
Summary
Responsible for ensuring the development, organization and coordination of living center and community resources to provide comprehensive Activity Services and programs that meet the needs and interests of each resident.
Essential job duties and responsibilities
Interview and assess all residents prior to the initial Care Plan Conference; document this information in the medical record, develop an individual recreation plan based on the assessment and participate in Interdisciplinary Care Plan meetings
Update assessments and plans as needed and required by state or federal regulations
Develop monthly recreation program calendars that reflect and meet the needs of facility residents
Communicate facility programs to residents, staff, family and volunteers
Manage facility Volunteer Program
Maintain departmental documentation that reflects services provided and resident progress towards goals
In coordination with social services facilitate the residents in the organization and continued development of a Resident's Council
Make job assignments and set priorities
Serve as member of QAA committee

Qualifications
As required by state, currently licensed or registered in state of practice as Activities Director, Social Worker, Occupational Therapist
High school diploma or equivalent
CNA certification
Minimum two (2) years’ experience in social or recreational program within the last five (5) years
Minimum one (1) year full-time experience in health care activities program
Certification as Therapeutic Recreation Specialist, preferred
Must be capable of maintaining regular attendance
KNOWLEDGE, SKILLS, ABILITIES & BEHAVIORS
Must be able to maintain confidentiality regarding patient and company proprietary information
Must have the ability to relate professionally and positively and work cooperatively with patients, families, and other employees at all levels
Physical and Sensory. Requirements (With or Without the Aide of Mechanical Devices ): Walking/mobility, reaching, bending, lifting, transferring and repositioning residents, grasping, pushing and pulling, fine hand coordination; ability to distinguish smells, tastes and temperatures; ability to hear and respond to pages; ability to read and write; ability to understand and apply training and in-service education; ability to instruct other staff in training education and staff meetings; ability to communicate with residents and others; and ability to remain calm in emergency situations and when handling multiple tasks.
