For more than 25 years, we've helped older adults and their families navigate the aging journey through professional Care Management and Home Care services. Our mission is simple: prevent falls, reduce hospitalizations, and help seniors aging wisely at home.
We're looking for someone who is passionate about helping seniors while also building relationships throughout the community. This is a unique opportunity to combine direct client care with business development as we continue expanding our Care Management services throughout Hillsborough County.
About the Role
This position starts as a hybrid role.
Initially, you'll spend part of your week providing Care Management services to existing clients and supporting our Pinellas County team. The remainder of your time will be spent building relationships with physicians, elder law attorneys, financial planners, hospitals, senior living communities, and other professionals throughout Hillsborough County.
As your referral network grows, you'll begin managing your own Hillsborough County clients. Over time, your schedule will naturally transition from community outreach to a full Care Management caseload.
If your caseload ever decreases, you'll return to developing referral relationships until your client hours are back to full productivity.
What You'll Do: Care Management
- Conduct comprehensive client assessments.
- Develop individualized care plans.
- Coordinate medical, community, and long-term care services.
- Advocate for clients and families.
- Attend physician appointments when appropriate.
- Monitor client well-being and adjust care plans as needs change.
- Communicate regularly with families and healthcare professionals.
- Complete documentation accurately and on time.
Relationship Development
- Build relationships with physicians, hospitals, rehabilitation centers, elder law attorneys, financial planners, trust officers, and senior living communities.
- Educate professionals on the benefits of Care Management.
- Schedule and conduct educational presentations.
- Attend networking events throughout Hillsborough County.
- Serve as an ambassador for EasyLiving within the community.
- Help grow our Care Management practice through relationship-based referrals.

- Comfortable building professional relationships.
- Confident meeting with physicians and referral partners.
- Self-motivated and organized.
- An excellent communicator.
- Compassionate while remaining professional.
- Able to work independently and solve problems.
Preferred Qualifications
- RN, LPN, Social Worker, Occupational Therapist, Physical Therapist, or related healthcare background.
- Experience working with older adults.
- Knowledge of Medicare, Medicaid, and community resources.
- Strong organizational and documentation skills.
- Valid Florida driver's license and reliable transportation.
