Duties
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using tools like Hoyer lifts and other assistive devices
- Monitor patient health by taking vital signs such as blood pressure, pulse, temperature, and respiration; document medical records accurately using Electronic Health Record (EHR) systems
- Support medication administration following proper protocols and ensuring compliance with HIPAA regulations to safeguard patient privacy
- Observe and report changes in patient condition, including behaviors related to dementia or Alzheimer's disease, to healthcare professionals promptly
- Provide companionship and emotional support while engaging patients in activities that promote mental well-being and social interaction
- Assist with light housekeeping tasks such as cleaning, laundry, meal preparation, and maintaining a safe environment in accordance with health standards
- Document all care activities thoroughly in medical documentation systems and ensure compliance with healthcare regulations
Experience
- Prior experience working in home health care, nursing homes, assisted living facilities or long-term care environments preferred
- Demonstrated ability to work with individuals with disabilities or developmental challenges; experience with hospice or memory care is a plus
- Knowledge of medical terminology and procedures related to patient observation, catheter care, medication administration, and hospice support
- Familiarity with EMR (Electronic Medical Records) or EHR systems for accurate documentation of patient information
- Certification in CPR (Cardiopulmonary Resuscitation) required; additional certifications such as HIPAA training or dementia care are advantageous
- Proven ability to handle medical equipment including Hoyer lifts and other mobility aids safely and effectively
- Strong communication skills to collaborate effectively with healthcare teams and families
