Hospice RN Case Manager
The Hospice RN Case Manager plays a vital role in coordinating care and supporting patients and families coping with terminal illness. You will perform comprehensive assessments, manage symptoms, educate families, collaborate with the interdisciplinary team, and ensure exceptional end-of-life care.
Key Responsibilities
- Coordinate, deliver, and evaluate individualized hospice nursing care.
- Complete initial and ongoing assessments, including physical, psychosocial, spiritual, and bereavement needs.
- Collaborate with the Interdisciplinary Group (IDG) to develop and follow the plan of care.
- Supervise tasks delegated to Hospice Aides, LVNs/LPNs, and Volunteers.
- Provide education to patients, families, caregivers, and referral sources on disease processes, symptom management, and end-of-life care.
- Document all care accurately and in real time.
- Communicate effectively through written and verbal reports and shift handoffs.
- Participate in weekly team meetings, patient care meetings, and bereavement support activities.
- Participate in on-call rotation and provide coverage during evenings, weekends, and holidays as needed.
- Assist with QAPI activities, data collection, and performance improvement projects.
- Represent Sacred Mission Hospice professionally and maintain a strong focus on patient and family experience.
Qualifications
- Current Registered Nurse (RN) license in the State of Texas (Required).
- Minimum 2 years of nursing experience in hospice, home health, or community health within the past 5 years.
- Strong clinical assessment and communication skills.
- Compassionate, patient-centered approach to care.
- Reliable transportation with appropriate insurance.
- Access to a working phone (cell or landline).
- Spanish Speakers Highly Encouraged to Apply
Physical Requirements
- Ability to assist with lifting and transferring patients.
- Ability to lift more than 20 pounds.
- Ability to stand, walk, kneel, or crouch as needed.
- Adequate vision for close and distance work and the ability to adjust focus.
