Intake / Admissions Nurse
Responsibilities and Duties

Facilitate admissions for incoming Skilled Nursing patients (does not include direct patient care)

Verify and enter physician orders

Identify level of care needed

Receive and give verbal orders accurately

Review clinical documentation to determine Skilled Nursing placement

Exceptional interpersonal and customer service skills
-
Ability to communicate clearly with medical providers

Work closely with referring hospitals, physicians, skilled nursing facilities, patients, and families
-
Manage after-hours and weekend referrals (rotating on call)
Schedule
-
Monday – Friday (day shift)
-
Rotating weekends and on-call (approximately every 6 weeks)

Pay: $28.00 – $32.00 per hour DOE

Qualifications:
-
Current license as a licensed LPN/RN from the Utah State Board of Nursing

1-year Skilled nursing facility experience
-
Good verbal and written communication skills

Basic computer knowledge

Ability to effectively problem solve

Provide excellent customer service
-
Knowledge of Medicare, Medicaid and 10A criteria
