Home Health Care, Inc. is looking for an experienced Physical Therapist who wants to specialize in Start of Care admissions, using their clinical judgment and assessment skills to establish the foundation for successful home health episodes throughout the Twin Cities.
If you're the PT who enjoys figuring out the complicated cases, identifying what others may have missed, and creating the roadmap other clinicians will follow, this role puts those strengths front and center.
Premium Pay for Specialized Clinical Work
- $100 per weekday Start of Care visit
- $120 per weekend Start of Care visit
- Weekend admissions are optional and paid at a bonused rate
- No holidays
- Mileage reimbursement for eligible travel

Weekend admissions are optional, allowing you to take advantage of the bonused rate when it works for your schedule without committing to a required weekend rotation.

This Isn't About Running Through a Treatment List
Start of Care requires a different skill set.
You'll meet patients shortly after hospitalization, surgery, illness, or a significant change in function and determine what needs to happen next.
You'll assess strength and mobility, but you'll also need to understand the bigger picture.
Can this patient safely transfer?
Are they at risk of falling?
Do they have the right equipment?
Can their caregiver safely assist them?
What disciplines should become involved?
What does the interdisciplinary team need to know immediately?
Your evaluation becomes the clinical starting point for everything that follows.
What You'll Be Doing
- Complete comprehensive Start of Care Physical Therapy evaluations in patient homes.
- Assess strength, balance, gait, endurance, transfers, mobility, fall risk, and functional limitations.
- Evaluate the patient's actual home environment for safety concerns and barriers.
- Complete OASIS and admission documentation accurately and within required timeframes.
- Develop individualized plans of care based on clinical findings, physician orders, and patient goals.
- Identify immediate safety concerns and recommend appropriate interventions.
- Recommend assistive devices and durable medical equipment when appropriate.
- Educate patients and caregivers on mobility, fall prevention, home exercise programs, and safe functional movement.
- Collaborate with OTs, SLPs, nurses, physicians, social workers, and other interdisciplinary clinicians.
- Communicate significant findings promptly to the appropriate care team members.
- Complete timely electronic documentation that supports quality and regulatory compliance.
We're Looking for More Than Treatment Skills
This opportunity is best suited for a PT who enjoys being the clinician responsible for putting the pieces together.
You'll need to walk into unfamiliar environments, establish rapport quickly, complete a thorough assessment, recognize clinical priorities, and confidently establish the direction of care.
You won't have someone standing next to you telling you what to assess next.
Your clinical judgment is why you're there.
Required Qualifications
- Current Minnesota Physical Therapist license in good standing
- Current CPR certification
- Valid driver's license and reliable transportation
- Willingness to travel throughout an approximately 75-mile radius as referral needs vary
- Strong assessment and critical-thinking abilities
- Excellent communication and documentation skills
- Ability to independently manage a field-based schedule
- Ability to collaborate effectively with an interdisciplinary clinical team
Preferred Qualifications
- Previous home health experience
- OASIS experience
- Start of Care / admission experience
- Knowledge of Medicare home health regulations
- Experience with orthopedic, neurological, cardiac, pulmonary, geriatric, or medically complex patients
