Peachtree Care Health Services is seeking a compassionate and dependable Mobile Registered Nurse (RN) to support our growing home care operations. The Mobile RN will travel to clients’ homes to conduct initial nursing assessments and supervisory visits to ensure quality care and compliance with Medicaid home care program requirements.
This role is ideal for an experienced nurse who enjoys working independently in the field while supporting caregivers and ensuring high-quality client care.
Key Responsibilities
- Conduct initial nursing assessments for new home care clients to determine care needs and eligibility for services.
- Perform supervisory visits to monitor caregiver performance and ensure quality of care.
- Develop and update individualized care plans based on client needs and physician orders when applicable.
- Evaluate client health status and report changes or concerns to the care team.
- Provide guidance and clinical support to caregivers regarding client care and safety.
- Ensure documentation is accurate, timely, and compliant with Medicaid and agency policies.
- Maintain communication with the administrative team regarding client status and service updates.
- Participate in quality assurance and compliance activities as required.
- Travel to client homes within assigned service areas.
Qualifications
- Active Registered Nurse (RN) license in the State of Georgia
- Minimum 2 years of nursing experience (home care experience preferred)
- Strong assessment and clinical documentation skills
- Ability to work independently in a mobile/field setting
- Reliable transportation and valid driver’s license
- Strong communication and organizational skills
Preferred Experience
- Home Health or Home Care experience
- Experience with Medicaid waiver programs such as CCSP, SOURCE, ICWP, or other community-based services
