Job Summary
We are seeking a passionate and experienced PRN Hospice Field Registered Nurse to join our team. Aspen Grove Hospice is a mission-driven hospice dedicated to providing the highest quality of compassionate care to patients and their families during the end-of-life journey. We employ the following systems to assist in our patient care: Relias learning management & CE platform, MatrixCare EMR, and CareXM triage and on-call system.

Duties
- Provide holistic nursing care to in-home hospice patients, including symptom management, medication administration, and comfort measures.
- Conduct thorough patient assessments to evaluate physical, emotional, and psychosocial needs.
- Collaborate closely with physicians, social workers, chaplains, and other healthcare professionals in the IDT to develop and implement individualized care plans.
- Manage care documentation and charting accurately and timely using Electronic Medical Record (EMR) systems while adhering to HIPAA regulations.
- Educate patients and families about disease progression, medication use, and end-of-life choices with clarity and compassion.
- Respond promptly to urgent patient needs through intake assessments and clinical case management.
- Possibly provide 1-5 days of On-Call per month if desired (for additional compensation).
Requirements
- Valid RN license with current state certification.
- Minimum of 2 years hospice experience within the past 5 years is required.
- Proven proficiency in using EMRs and documentation practices compliant with HIPAA; experience with MatrixCare is a bonus.
- Excellent patient service skills with the ability to communicate effectively with diverse populations and their families.
- Ability to manage multiple priorities in high-pressure situations while maintaining professionalism and compassion.
- Valid Driver's License and auto insurance.
- Must pass background checks.

Pay: $42.00 - $48.00 per hour
