American Traveler is hiring a Home Health RN with HCHB and OASIS charting experience for a field position requiring on-call rotation and same-day patient visits.

Field-based Home Health RN role providing in-home patient care within a designated service territory
-
Day shift schedule, Monday through Friday, 8:00 AM to 5:00 PM
-
Average of 4-6 patients per day, with a minimum requirement of 30 points per week
-
Patient population includes adult and geriatric home care cases
-
Responsible for interdisciplinary care coordination and plan of care development and oversight
-
Charting must be completed using HCHB and OASIS while at the patient's home
-
Weekend coverage required as needed

Participation in on-call rotation required, including at least one weeknight per week

Must be willing to travel up to 50 miles outside the home base office

Active TX or compact RN license required, must be in hand at time of consideration

Minimum 1 year of Home Health nursing experience required

Minimum 2 years of overall clinical nursing experience required

BLS certification required
-
HCHB and OASIS charting experience required

Valid driver's license required

Must have reliable transportation and insurance

Non-smokers preferred due to patient sensitivity to smoke odor

Responsible for administering skilled nursing care to patients from diverse populations across a variety of home environments

Coordinates care with patients, families, certifying providers, and interdisciplinary team members

Performs comprehensive patient assessments and collaborates on plan of care development

On-call responsibilities include taking after-hours calls and completing related charting
-
Dress code consists of scrubs in any color
