Rapid Rehousing Case Manager Position Overview and Job Duties
This position entails the following responsibilities and may include other duties as assigned:
- Provide case management services including intakes, assessments, service planning, referral and linkage management, and advocacy to a caseload of 25 individuals or families who are homeless.
- Identify client needs and conduct bi-monthly home visits.
- Perform crisis prevention and interventions as needed using Harm Reduction & Trauma informed Philosophies and act as a liaison between landlords/property managers and clients.
- Assist clients in meeting all lease requirements including documentation and income certification.
- Work with clients to assist them in addressing issues to prevent eviction. Work with the landlords to resolve landlord/client’s disputes.
- Coordinate moving, furniture, and starter kit resources.
- Assist clients with rehousing should they require a move before their lease is up and/or at the time their lease is up.
- Ensure clients are receiving all necessary covered health insurance benefits including medical and behavioral health services with regard to many of the services our agency currently provides.
- Ensure clients respond to requests to verify eligibility as part of the State’s Medicaid redetermination process in a timely manner so as not to lose medical coverage.
- Make appropriate referrals for services based on client goals and needs and ensure that linkages are secure and functional.
- Provide education and resources based on the needs of clients
- Assist clients in enrolling in public benefits such as SNAP, SOAR or employment readiness programs.
- Assist clients in obtaining state ID, social security card, birth certificate, and other necessary documents to receive services.
- Verify enrollment in medical care or support clients to engage in medical care by providing information, assisting in finding medical providers or helping clients make appointments.
Key Responsibilities
- Conduct initial intake and assessment to determine client eligibility for the program.
- Develop individualized housing plans based on client needs, strengths, and goals.
- Provide case management services, including resource referrals for employment, education, and mental health support.
- Assist clients in navigating rental agreements and financial responsibilities.
Housing Placement and Retention
- Identify and secure suitable housing opportunities for clients.
- Build and maintain relationships with landlords and property managers to expand housing options.
- Mediate landlord-tenant concerns to promote positive relationships and housing retention.
Financial Assistance and Reporting
- Assist clients in accessing rental assistance, utility payments, and other housing-related resources.
- Track and document client progress, housing stability, and program outcomes using case management software.
- Prepare regular reports for internal and external stakeholders, ensuring compliance with funding requirements.
Skills and Competencies
- Strong understanding of housing-first principles, rapid re-housing, and trauma-informed care.
- Excellent interpersonal, problem-solving, and conflict-resolution skills.
- Knowledge of local housing markets, tenant rights, and eviction prevention strategies.
- Ability to manage multiple cases and maintain accurate, confidential records.
Technical Requirements
- Proficiency in case management systems and office software (e.g., MS Office).
- Valid driver’s license and reliable transportation for field visits.
- Experienced in using Zoom, Teams, Skype, and other virtual meeting platforms.
- Willing to learn and use new software and databases as needed, including FYI.
Time management
- Excels at time management and organization.
- Demonstrates competence in task prioritization and resourcefulness.
- Capable of balancing multiple demands concurrently.
- Supports staff with time efficiency to meet tight deadlines.
