Overview:
Provides Information, Skills Training, Peer Support, and Advocacy to persons with disabilities empowering them to achieve personal goals towards living independently in the community as they choose.
Responsibilities:
- Serve to provide and coordinate services on behalf of consumers as assigned and with a consistent emphasis on self advocacy, individual empowerment and responsibility.
- To meet in person with individual consumers and groups in their homes, schools, workplaces or community setting of their preference.
- Demonstrate ability to assist consumers in the assessment of skills assets/deficits and needs for independent living and/or other services.
- Assist consumers in the development, implementation, and monitoring of independent Living Plans designed to address consumer identified goals.
- Provide skills training and/or peer counseling to consumers in individual and group settings.
- Provide direct advocacy training/support on behalf of the human and civil rights of persons with disabilities.
- Coordinate activities with service providers as needed with the support of the Independent Living Programs & Services Coordinator.
- Document work through the use of narrative and statistical forms.
- Maintain strict confidentiality and professional behavior at all times in working with consumers and colleagues.
- Attend consumer review and staff meetings; other duties as required.
Minimum Qualifications:
- Ability to work both in person with consumers and community partners.
- Bachelor degree in Human Services or like field preferred, demonstrated professional experience will be considered.
- Excellent organizational, problem solving/planning and communication skills.
- Ability to establish and maintain productive relationships with consumers, co-workers and supervisor in addition to working as a team player.
- Knowledge of Microsoft Office suite and demonstrated computer abilities.
- Strong interpersonal, analytic and communication skills
- Demonstrated ability to manage multiple projects and priorities; attention to detail
- Lived or direct experience with disability highly desirable
- Legally registered and insured vehicle required.

Pay: From $21.00 per hour
