Job Summary
We are seeking a compassionate and dedicated Home Care Aide to provide personalized in-home support for clients with diverse needs on a "PER DIEM/AS-NEEDED" basis This vital role involves assisting individuals with daily activities, ensuring their safety, and promoting independence in their living environments. As a Home Care Aide, you will play a crucial part in enhancing the quality of life for seniors, individuals with disabilities, or those recovering from illness, all while adhering to home healthcare regulations and maintaining patient confidentiality. Your energetic approach and attention to detail will make a meaningful difference in our clients' lives.
Duties
- Provide personal care assistance including bathing, dressing, grooming, and mobility support using patient transfer methods and equipment such as Hoyer lifts when necessary
- Assist with Activities of Daily Living (ADLs) such as toileting, feeding, and medication reminders while ensuring proper documentation within Electronic Health Record (EHR).
- Monitor patient health by observing vital signs, recognizing signs of medical emergencies like Alzheimer's or dementia-related behaviors, and reporting changes promptly
- Support clients with meal preparation, light housekeeping, laundry, and cleaning to maintain a safe and comfortable environment
- Medication Reminders following medical terminology guidelines and home care protocols while maintaining HIPAA compliance and patient confidentiality
- Document medical records accurately, including notes on patient observations, medication administration, and care provided
- Assist with mobility and transfers using safe patient transfer techniques to prevent injury for both client and caregiver
Experience
- Prior experience working in assisted living facilities, nursing homes, or home healthcare agencies is highly preferred
- Demonstrated knowledge of dementia care, Alzheimer’s care, hospice care, and geriatrics
- Experience working with individuals with developmental disabilities or disabilities requiring specialized caregiving techniques
- Familiarity with EMR/EHR systems for medical documentation and client communication
- Certification in First Aid and Basic Life Support (BLS), including CPR training
- Understanding of home health care regulations and medical terminology related to patient care
- Proven ability to handle heavy lifting safely and operate equipment such as Hoyer lifts or patient transfer devices
