Make a Difference Where It Matters Most
Team Wellness Center is seeking compassionate and dedicated Home-Based Clinical Therapists to provide community-based mental health services to children, adolescents, and families. In this role, you'll meet clients where they are—providing therapy in homes, schools, and community settings while helping individuals overcome behavioral health challenges and build lasting stability.
As part of our multidisciplinary team, you'll collaborate with psychiatrists, case managers, nurses, care coordinators, and other behavioral health professionals to provide high-quality, person-centered care.
What You'll Do
- Conduct comprehensive biopsychosocial and behavioral health assessments
- Develop individualized, person-centered treatment plans
- Provide individual, family, and collateral therapy in home and community settings
- Deliver trauma-informed, evidence-based therapeutic interventions
- Complete crisis assessments and provide crisis intervention when needed
- Collaborate with community providers, schools, hospitals, and other treatment team members
Required
- Master's degree in Social Work or Counseling
- One of the following Michigan licenses:
- LLMSW or LMSW
- LLPC or LPC
- Valid Michigan driver's license
- Reliable transportation with the ability to travel throughout the community
- Ability to work independently while managing a community-based caseload
Preferred
- Experience providing therapy to children, adolescents, and families
- Experience with trauma-informed care
- Experience working with individuals with co-occurring mental health and substance use disorders
- CAADC certification or willingness to obtain
- Motivational Interviewing and person-centered treatment experience

Pay: $60,000.00 - $75,000.00 per year
