Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Monitor patient health by observing vital signs and recognizing changes in condition; accurately document medical information using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems
- Administer medications following prescribed schedules and medical terminology guidelines, ensuring proper documentation and adherence to home healthcare regulations
- Support clients with specialized care needs such as Alzheimer’s disease, dementia, memory care, hospice/end-of-life care, and caring for individuals with developmental disabilities or geriatrics
- Operate patient transfer methods safely using equipment like Hoyer lifts or patient transfer techniques to prevent injury
- Maintain patient confidentiality in accordance with HIPAA regulations and ensure privacy during all interactions
- Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe living environment for clients
- Assist with medical procedures such as catheter care or assisting with wound management when trained and authorized to do so
- Communicate effectively with clients’ families and healthcare teams to coordinate care plans and updates
- Follow all home health agency policies and home & community care standards to deliver consistent, high-quality service
Qualifications
- Proven experience working in assisted living facilities, nursing homes, or home healthcare settings; experience working with individuals with disabilities or dementia is highly valued
- Knowledge of medical terminology, medication administration protocols, vital signs monitoring, and basic life support (BLS) including CPR certification
- Familiarity with EMR/EHR systems for accurate medical documentation and record keeping
- Ability to perform heavy lifting safely when required; experience operating patient lifts such as Hoyer lifts is preferred
- Strong skills in patient personal care assistance, client communication, and maintaining patient confidentiality under HIPAA guidelines
- Valid driver’s license and reliable transportation are often required for client visits within the community
- Certification in first aid and CPR; additional certifications related to hospice or end-of-life care are advantageous
- Demonstrated ability to work independently while managing multiple client needs in a compassionate manner
- Knowledge of home health care regulations and safety standards necessary for delivering compliant services
