Registered Nurse (RN) – Dementia & Memory Care Specialist

Youthful Aging Home Care is seeking an experienced, compassionate Registered Nurse with a passion and expertise in dementia and memory care to join our growing private-duty home care team in Sarasota.
This is an opportunity for a special RN who understands that dementia care requires more than clinical knowledge. It requires patience, empathy, exceptional communication, creativity, and the ability to support both the client and the family through the challenges of cognitive decline.
The Role
As our RN Dementia & Memory Care Specialist, you will help lead and strengthen our clinical approach to caring for clients living with Alzheimer's disease, dementia, and other cognitive impairments.
You will work closely with clients, families, caregivers, physicians, and community resources to develop individualized care strategies that promote safety, dignity, comfort, engagement, and the highest possible quality of life.
Responsibilities
- Conduct comprehensive client assessments with a strong focus on cognitive status, safety, behavior, and changing care needs
- Develop individualized dementia and memory-care plans
- Provide ongoing clinical oversight and reassessment of clients
- Educate and support families navigating Alzheimer's disease and other forms of dementia
- Help families understand disease progression, behavioral changes, communication techniques, safety concerns, and care options
- Develop strategies for managing agitation, anxiety, wandering, sundowning, resistance to care, and other dementia-related behaviors
- Train and mentor caregivers in dementia-specific care techniques, communication, redirection, and person-centered care
- Participate in client admissions and care-plan development
- Collaborate with physicians, healthcare providers, families, and other members of the care team
- Monitor changes in client condition and recommend appropriate interventions
- Help ensure caregivers are properly matched to clients with dementia and memory-care needs
- Participate in quality assurance and clinical improvement initiatives
- Serve as a trusted clinical resource for families and staff
- Maintain accurate and timely clinical documentation
Qualifications
- Current, active, and unrestricted Florida Registered Nurse (RN) license
- Experience working with Alzheimer's disease, dementia, memory care, geriatrics, home care, assisted living, skilled nursing, hospice, or geriatric care management
- Dementia-specific training, certification required
- Experience developing care plans and conducting client assessments
- Strong communication and family education skills
- Private-duty home care experience preferred
- Supervisory or caregiver training experience preferred
- Valid driver's license, reliable transportation, and willingness to travel locally throughout Sarasota and surrounding communities
- Ability to successfully complete required background screening and other employment requirements
