- Willing to do Home health and Facility Visits
- Perform Start of Care , complete necessary paperwork and report findings and plan of care to ordering physician .
- Perform initial evaluation and complete necessary paperwork.
- Report plan of care to nurse case manager if not therapy case only.
- Turn in completed evaluations to Rehab Maxx within 24 hours .
- Perform follow up visits and complete necessary paperwork.
- Communicate any drastic change in patient's condition to physician or nurse case manager
- Supervise physical therapy assistant,
- Perform supervisory visit and complete necessary paperwork after 5 completed physical assistant visits .
- Perform 30th day Re assessment visits when required and complete necessary paperwork. Perform discharge visit and complete necessary paper work Communicate discharge to nurse case manager .
- Turn in completed visit notes and discharges to Rehab Maxx weekly.

Pay: $80,000.00 - $90,000.00 per year
