Registered Nurse Case Manager: Home Healthcare (Full-Time/Part-Time)
Seeking position for a full-time RN to fulfill a home healthcare Case Manager position. Locally owned and operated, Great Lakes Home Healthcare Specialists, LLC has been providing extraordinary care for nearly 20 years. Currently, the counties which we serve are Grand Traverse, Antrim, Benzie, Kalkaska, Leelanau, and Wexford.
This position is for Medicare skilled homecare services. Previous experience with OASIS documentation preferred!
Our RN Case Managers provide individualized nursing care to patients in their home. Post-hospitalization care management, wound care, medication education and patient/family education are among the services offered. As such, our company values providers who are driven, organized, and self-motivated. Professional communication and time-management skills are a must.
Case Managers are assigned “territories” for day-to-day operations, in hopes of decreased drive times for staff and provider consistency for patients (you may be required to travel outside your territory to cover for vacations and heavier patient volumes during peak season). We compensate for mileage, weekend visit bonuses, and offer quarterly bonuses based on productivity.

Requirements (not an all-inclusive list):
- Current, unrestricted Registered Nurse license with the Michigan Board of Nursing
- CPR/BLS certified
- Minimum one year of nursing experience (preferred)
- Demonstrable knowledge of care and communication skills with the geriatric, disabled, and functional adult populations
- Ability to use Electronic Medical Record (EMR) software
- Home Healthcare/OASIS experience (preferred)
- Reliable transportation
- Current driver’s license and auto insurance
