Mission Hills Dialysis LLC, a growing dialysis provider in the San Fernando Valley, is seeking an experienced and compassionate Peritoneal Dialysis Nurse (PD RN) to join our team. The Peritoneal Dialysis Nurse (PD RN) plays a key role in ensuring safe, effective, and compassionate patient care. This position works closely with the Medical Director, nursing team, clinical professionals, administrative staff, patients, and families to promote positive clinical outcomes and improve the overall dialysis experience.

Our Peritoneal Dialysis Nurse (PD RN) will lead by example, developing a team that consistently delivers individualized care while maintaining the highest standards of clinical quality, patient safety, and professionalism.
Patient Care & Peritoneal Dialysis
· Welcome patients and provide compassionate, professional, and respectful care.
· Perform and document comprehensive nursing assessments and develop individualized care plans based on patient needs, physician orders, and dialysis prescriptions.
· Provide peritoneal dialysis training and ongoing support to patients and caregivers, including treatment procedures, equipment use, infection prevention, and safe home practices.
· Assess and provide care for PD catheters and exit sites, monitoring for infection and other complications.
· Monitor treatment effectiveness, fluid status, ultrafiltration, blood pressure, weight trends, and other relevant clinical parameters.
· Evaluate PD adequacy and identify, assess, and respond appropriately to complications including peritonitis, catheter problems, fluid imbalance, and treatment-related concerns.
· Administer medications and treatments as ordered and within the RN scope of practice.
· Obtain laboratory specimens, review results, and follow up on abnormal findings.
· Respond appropriately to emergencies and communicate significant clinical changes to the physician and interdisciplinary team.
· Perform Home visits/consults. Promote patient independence, safety, comfort, dignity, and confidence in managing therapy at home.
Infection Prevention, Documentation & Compliance
· Maintain strict infection prevention standards, including hand hygiene, aseptic technique, PPE, catheter care, and Standard Precautions.
· Educate patients and caregivers on infection prevention, catheter care, proper handling of supplies, and recognition of peritonitis and other complications.
· Accurately document assessments, treatments, education, interventions, patient responses, training, and follow-up in the EMR.
· Maintain compliance with State and Federal regulations, CMS Conditions for Coverage, facility policies, and applicable clinical standards.
· Participate in QAPI, audits, patient care planning, regulatory activities, and quality improvement initiatives.
Patient Education & Care Coordination
· Provide individualized education regarding PD, medications, fluid management, diet, catheter care, infection prevention, and treatment adherence.
· Assess patient and caregiver competency and provide additional training or support as needed.
· Identify barriers to successful home dialysis and work with patients and caregivers to develop solutions.
· Collaborate with nephrologists, dietitians, social workers, nurses, patients, caregivers, and other members of the interdisciplinary team.
· Participate in patient rounds, care planning, staff meetings, in-services, and continuing education.
· Serve as a clinical resource for patients, caregivers, and the healthcare team.
Additional Responsibilities
· Maintain patient confidentiality and comply with HIPAA requirements.
· Follow all company policies, procedures, safety requirements, and applicable healthcare regulations.
· Assist with PD admissions, transitions to home therapy, emergency preparedness, and other clinical or operational needs.
· Maintain required licenses, certifications, and competencies.
· Perform other duties as assigned.
· Scheduling: Alternative work schedules may be available based on facility and operational needs.
Qualifications
· Current California Registered Nurse (RN) license in good standing required.
· Current BLS/CPR certification required.
· Peritoneal dialysis or home dialysis experience strongly preferred.
· Knowledge of PD modalities, catheter care, infection prevention, and dialysis-related complications.
· Strong patient assessment, clinical judgment, teaching, and communication skills.
· Ability to educate patients and caregivers on complex clinical procedures in a clear and understandable manner.
· Strong organizational and time-management skills with the ability to work independently.
· Proficiency with EMR/EHR systems and clinical documentation.

Pay: $37.00 - $47.00 per hour
