Regional MDS Nurse (RN/LPN) – Full Time

JAG Healthcare is currently seeking a detail-oriented, organized, and compassionate Regional MDS Nurse (RN/LPN) to support our Salem and Orrville skilled nursing facilities. This is an excellent opportunity for a nurse who enjoys clinical assessment, interdisciplinary collaboration, and improving resident outcomes through accurate documentation and person-centered care.

This position supports two of JAG Healthcare’s close-knit skilled nursing communities, allowing you to build strong relationships with residents and staff while enjoying the variety of working alongside two collaborative leadership teams. You will partner closely with each facility’s Administrator, Director of Nursing, therapy team, and interdisciplinary staff to ensure accurate MDS completion, regulatory compliance, and exceptional resident care.

Responsibilities
(include but are not limited to):
- Complete and coordinate resident MDS assessments, including admission, quarterly, annual, and significant change assessments
- Develop, review, and maintain individualized, person-centered care plans in collaboration with the interdisciplinary team
- Coordinate assessment schedules and ensure timely completion and submission of all required MDS documentation
- Collaborate with nursing, therapy, dietary, social services, activities, physicians, and other interdisciplinary team members to gather assessment information
- Monitor resident clinical status and identify changes that may require reassessment or care plan revisions
- Ensure accurate MDS coding in accordance with CMS, Medicare, Medicaid, PDPM, and state regulations
- Review documentation to support reimbursement accuracy, quality measures, and case mix optimization
- Lead and participate in interdisciplinary care conferences while collaborating with residents, families, and the healthcare team to develop person-centered care plans
- Educate and support clinical staff regarding MDS processes, documentation requirements, reimbursement, and regulatory compliance
- Participate in Quality Assurance and Performance Improvement (QAPI) initiatives to support positive clinical outcomes and continuous quality improvement
Skills & Qualifications
- Current Ohio Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in good standing required
- Previous MDS Coordinator or MDS assessment experience preferred
- Long-term care or skilled nursing experience preferred
- Knowledge of Medicare, Medicaid, PDPM, CMS regulations, reimbursement systems, and case mix preferred
- Strong organizational, analytical, and critical thinking skills
- Excellent written and verbal communication abilities
- Ability to prioritize multiple deadlines while maintaining exceptional attention to detail
- Ability to work independently while collaborating effectively with interdisciplinary teams
- Reliable transportation and willingness to travel between the Salem and Orrville facilities
