Primary responsibilities:
- Comprehensive psychosocial assessments
- Developing and maintaining a plan of care in collaboration with the team
- Coordinating dynamic activity programs and documentation on the unit
- Resource referral and contact with families of residents to provide support and/or collaboration towards goals of the family and resident.
- Crisis and therapeutic interventions
- perform assigned resident interviews that assess each individual’s mood, behavior, and cognitive ability and write the accompanying summary of findings.
- Knowledge, skills and abilities:
- Provide education and techniques to unit staff of person-centered interventions that can be utilized to address any mood, behavioral, or cognitive concerns residents might present with.
- Meets deadlines for time sensitive responsibilities.
- Excellent follow through and communication skills
- Ability to work with charge nurse to lead the Unit team.
- Passion for the elderly
- Ability to engage with families, residents and team.
- Qualifications:
- Degree in Social work or related field of study
- 2 years' experience in Human Services or applicable field
- Prior experience in activities a plus
- Ability to manage moving priorities each day
- Strong verbal and written communication skills
- Computer skills for charting and documentation
- Strong team player with the ability to have FUN!

Pay: $58,000.00 - $62,000.00 per year
