Part-Time (30 hours/week, 0.75 FTE) | Saint Joseph, MI | $55,000 - $60,000 per year

About the Role
Region IV Area Agency on Aging (RIV AAA) is seeking a CHW Integration & Workforce Lead to support the Southwest Michigan Regional Hub.
This role strengthens Community Health Worker (CHW) workforce development, training, integration, and sustainability across healthcare, public health, education, and community partners.
The Hub connects healthcare providers, public health agencies, and community organizations to improve care coordination and access to services throughout Berrien, Cass, and Van Buren Counties.
Responsibilities
- Lead CHW workforce development and regional strategy.
- Coordinate CHW training, credentialing, continuing education, and partner support.
- Strengthen CHW integration with healthcare, public health, education, and community partners.
- Support shared workflows, referral processes, data, reporting, and evaluation.
- Facilitate workgroups and help partners move initiatives from planning to implementation.
Qualifications
- Five (5) years of experience in CHW programs, human services, workforce development, or related fields.
- Community Health Worker certification required.
- Experience leading programs, projects, workgroups, or multi-partner collaborations.
- Experience working with healthcare organizations, community-based organizations, public health agencies, educational institutions, or related fields.
- Bachelor's degree in Human Services or other related field preferred.
