Social Director / Long Term Care Facility.
Job duties include, completing admission paperwork, notifying Office of Long Term Care of new admissions, Coordinating Care Plans for both skilled and Long Term care Residents along with scheduled admit and quarterly assessment documentation. Discharge planning along with Medicare nurse for skilled residents to include setting up Home Health and assisting with any medical equipment needed at home.
This position requires you to be detailed oriented and a self starter. As the Social Director you are a point of contact for both residents and their family.
The job is Mon-Friday 8-4:30, but must be flexible as we sometimes have late admissions or weekend admissions.
Experience in Long Term Care Social Service is preferred.
