Job Summary
We are seeking a dedicated and skilled Physical Therapist to join our dynamic healthcare team. The ideal candidate will possess a strong background in physiotherapy practices, with experience in outpatient and inpatient settings. As a vital member of our team, you will develop personalized care plans, deliver exceptional patient care, and contribute to the recovery and well-being of diverse patient populations, including geriatric, pediatric, neurological, and orthopedic cases.
Responsibilities
- Assess patients’ physical conditions through detailed evaluations, including physiology and anatomy knowledge.
- Design and implement individualized treatment plans tailored to each patient’s needs, utilizing techniques such as trigger point therapy and other physiotherapy modalities.
- Manage discharge planning processes to ensure seamless transitions from hospital or inpatient settings to home or community care environments.
- Document clinical findings accurately in electronic health records (EHR) systems while adhering to clinical documentation standards.
- Collaborate with multidisciplinary teams to coordinate patient care across outpatient clinics, hospitals, home health services, and rehabilitation centers.
- Provide education to patients and caregivers on injury prevention, exercises, and recovery strategies.
- Monitor patient progress regularly and adjust treatment plans accordingly to optimize outcomes.
- Maintain compliance with healthcare regulations and uphold high standards of safety and hygiene in all clinical activities.
Requirements
- Valid licensure as a licensed Physical Therapist with practicing credentials in relevant jurisdictions.
- Proven experience working with geriatric patients, pediatrics, neurology, orthopedics, or outpatient rehabilitation centers.
- Strong understanding of medical terminology, physiology, anatomy, and clinical documentation standards.
- Experience managing care plans for diverse populations in outpatient clinics or hospital settings.
- Familiarity with electronic health records (EHR) or electronic medical records (EMR) systems for documentation purposes.
- Demonstrated ability to work effectively within multidisciplinary teams across inpatient and outpatient environments.
- Knowledge of discharge planning procedures and community-based care strategies.
- Excellent communication skills combined with a compassionate approach to patient care.

Pay: $62,000.00 - $125,000.00 per year
