About the Role Optum Care at Home is seeking a Nurse Practitioner or Physician Assistant to provide longitudinal, in-home care for high-risk health plan members. Providers work within an interdisciplinary care team to manage complex medical conditions, improve health outcomes, and help patients remain safely in their homes.
Key Responsibilities
Conduct comprehensive in-home assessments, including physical exams, medication reviews, and screenings
Manage acute illnesses and chronic disease exacerbations
Support members during short-term rehabilitation episodes
Ensure accurate documentation and ICD-10 coding
Collaborate with interdisciplinary care teams
Conduct advanced illness and advance care planning discussions
Educate patients and caregivers
Travel extensively throughout Washington to provide home-based care
Required Qualifications
NP: ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
PA: NCCPA certification and active state licensure
Active, unrestricted license (or ability to obtain)
Active DEA license/prescriptive authority (or ability to obtain)
Valid driver's license and reliable transportation
Ability to obtain a collaborative practice agreement if required
Ability to perform physical assessments in home-based settings
Preferred Qualifications
1+ years of APC experience
Experience in geriatrics, home health, senior living, or long-term care
Experience caring for medically complex patients
Knowledge of chronic illness, geriatrics, advanced illness, and end-of-life discussions
EMR and technology proficiency
