Job Summary
Duties and Responsibilities:
- Shall be the case manager in all cases involving nursing and therapy care.
- Be responsible for the clinical record of each patient receiving care under his/her responsibility.
- Assure that progress reports are made to the physician for patients under medical care when the patient’s condition changes and are derivations from the plan of treatment.
- Responsible for following physician’s orders and reporting changes in patient’s condition to physician.
- Makes scheduled visits and regularly re-evaluates the patient’s nursing needs, following the plan of treatment and revision when necessary.
- Teaches, supervises, and consults with the patient and family members in methods of meeting the nursing care needs and other related problems.
