We are seeking a Registered Dietitian to join an outpatient PACE (Program of All-Inclusive Care for the Elderly) facility in South Los Angeles. This temporary opportunity is ideal for a Dietitian who enjoys working with a geriatric population and is looking for a consistent Monday–Friday, 8:00 AM–5:00 PM schedule in an outpatient setting.
As part of the interdisciplinary team, the Registered Dietitian will provide comprehensive nutrition services to older adults, including nutritional assessments, individualized care planning, therapeutic diet management, nutrition education, and ongoing monitoring of participants’ nutritional needs.
Key Responsibilities
- Complete initial, annual, semi-annual, and unscheduled nutritional assessments and reassessments.
- Develop and implement individualized nutritional plans of care in collaboration with the interdisciplinary team (IDT).
- Evaluate nutritional status, dietary needs, food intake, and the effectiveness of therapeutic diets and nutritional supplements.
- Monitor and manage special diets and communicate nutritional concerns and recommendations to the IDT.
- Provide nutrition education to participants, families, caregivers, and staff.
- Participate in meal rounds and monitor food quality, menus, sanitation, and nutritional adequacy.
- Assess participants’ access to food and make appropriate referrals for services such as Meals on Wheels, grocery assistance, and meal preparation support.
- Participate in wound care meetings, family meetings, QAPI initiatives, staff meetings, and department projects.
- Collaborate with in-home service providers regarding participants’ nutrition, food intake, meal preparation, and kitchen safety as needed.
- Assist with meal service and monitor dietary supplies, inventory, and vendor relationships.
- Supervise and provide guidance, training, and support to dietary staff.
- Maintain accurate documentation and ensure compliance with applicable policies, procedures, and safety standards.
- Maintain participant confidentiality and provide professional, compassionate service to participants and their families.
- Perform other duties as assigned.
Qualifications
- Bachelor’s degree from an accredited college or university in Nutrition, Dietetics, Food Science, or a related field.
- Registered Dietitian (RD/RDN) credential through the Commission on Dietetic Registration (CDR).
- Minimum 1 year of experience working with frail, elderly, or geriatric populations.
- Strong knowledge of clinical nutrition and therapeutic diets.
- Ability to assess nutritional needs and develop appropriate nutrition care plans.
- Knowledge of medical terminology and clinical documentation.
- Strong communication and interpersonal skills.
- Ability to work effectively as part of an interdisciplinary healthcare team.
- Must meet applicable health clearance and immunization requirements for direct participant care.
